We accept MOST Health & Vision plans:
Many Health plans offer routine vision exams every 2 years. If you are not elegible for a routine vision exam AND you are experiencing vision problems (such as blurry vision or eyestrain), you may obtain a referral from your Primary Care Physician and have your exam covered by your insurance as a medically necessary visit. Please contact our office if you need assistance with your insurance.
If you have insurance for which we are not a provider, we will be happy to provide you with a billing summary, which you may use to submit to your insurance company for reimbursement.
Please be aware of any co-payments and Deductibles that you may be responsible for according to your insurance contract.
